A Silver Alert has been issued in Waukesha County for a man who was last seen Friday afternoon at his assisted living facility in Brookfield.

Silver Alerts are issued for missing elderly people who are believed to have dementia or other cognitive impairment.

Thomas P. Rezutek, 72, was last seen around noon Friday at the assisted living facility at 18740 W. Bluemound Road, according to the alert from the Wisconsin Department of Corrections and Brookfield police.

Rezutek was described as 5 feet 7 inches tall, weighing 220 pounds, with brown eyes and partially gray hair. He was last seen wearing a black jacket, blue jeans and black shoes.

According to the warning, Rezutek does not have access to a vehicle or a cell phone. His destination and the reason for his departure are unknown.
